Especially a web form with a 1"x2" box for my message, and with no
indication that you will provide me with a copy.
Common scenario: "We want your feedback" -- but apparently "we" only
want three or four words of feedback, no more.
I don't mind retyping your email address if you want to write, say,
<edward(at)paleo.org>. But I agree with Steve: I won't use a web
feedback form unless it's really important to me, and even then I
usually just use it to ask for an email address.
