I've found that Netcom's mail-to-news gateway can cause duplicate posts:
apparently it doesn't like the symbol "/" in Message-IDs and changes it
to "_". Since the resulting Message-ID is different, the post is mailed
again.

I've had this problem when posting using the server at rtfm.mit.edu.
