The footer I do, at least halfway.

"How do I get off?" is FAQ #1 on any list I have been on where
something akin to this footer hasn't been the list practice.

The other weirdness is probably because newbies use HotMail etc.
to read their mail by POP3 and don't actually remember what their
email address is that they subscribed to the list from (nor do
they know how to read it in the headers).

I am not too sure about the latter. I'm not subscribed to
the list-managers list any more.
