I have a dog. She had puppies last year and had 4 but one...

I have a dog. She had puppies last year and had 4 but one was not fully developed so tech. 3. This year she had another litter and only had 2. We were very surprised. The puppies are two days old and I noticed that she is still tired.  She wasn't like this last litter. She is being a good mom though. She also threw up some green stuff today. I looked closer to her and she has mucusy eyes, nose.  Has anyone ever experienced this before. She is only 5 lbs. and has a buldge in her stomach, is it possible for another puppy to be in there?
