Far and away your best bet is to use the posting server at
rtfm.mit.edu. Not only will it add the needed headers, it will get them
right every time (which is a lot harder to do manually than it sounds),
and will post it on schedule even when you are on vacation!

I've been using it for about 7 years. At first I wondered about my loss
of control over exactly what day my FAQ got posted. I got over that in
a hurry. It just doesn't matter. I update when I update, it posts when
it posts, and the readers are happy ...

get this file by sending a message to faq-server@rtfm.mit.edu with HELP
as the subject.

As it's been a while since your original approval, you may also want to
review the post-approval guidelines at
ftp://rtfm.mit.edu/pub/faqs/news-answers/postapproval-guidelines so you
remember what changes require approval and -- more importantly from the
"FAQ of FAQs" point of view -- which don't require approval.
