I have a utility (a perl script) I wrote which does this. (It also
does a fair bit of specialized massaging of the files for my FAQ, but
I could easily take that out.) If you're interested, within a few
days I could probably clean it up, document it, and send you a copy.

I'd also be willing to make the script a bit more general, if a couple
of people could agree on a format for it to look for. I should warn
you, though, there's still a fair amount of work involved in creating
the "source" version of your FAQ for this script to chew on.
