Okay, when a creature attacks and another blocks, one dies.  But what about the other, do you put "counters" of any kind on it to show damage or is it like new the next turn?

This is what the cleanup step is for. A creature will remember how much damage is on it until the cleanup step, where all damage is removed from creatures. This happens at the end of each players turn, and removes damage from any creature still alive on either side.

This basically means that if you got a 5/5 in play, and then cast earthquake for 2, then attack, and it gets blocked by a 3/3, it will die. But if you don't attack, the 2 damage goes away at end of turn.
