My dachund is trained to a large litter box. She uses in for about three months and then uses the bathroom in the floor. What causes her to do this?

maybe the litter box has an unberable smell that only she smells

I, just got my dacshound two days ago. (5 months old) . she has never had any type of training.  I, would like to start housebreaken her. I, take her outside every two hours.and she won't go to the bathroom,and when you bring her back in she pee's  Help
