I have a German Shepard and he's 2 yrs old he's having alot...

I have a German Shepard and he's 2 yrs old he's having alot of pain in both ears they look dry and sometimes there are small red sores, we clean his ears atleast once a week with sea breeze, now the sores are gone but he still crys when he scratches his ears what can cause this? and how do we help him?  The vet said it was an inner ear problem and gave us medication but this never helped and its very nocticable on his ears.  Please can you give us some advice so our beloved dog can stop hurting.. THANK YOU
