When you first get your belly button pierced, can it be done with a chain? or does it have to be with a barbell type jewelry?

it's usually done with a bar bell so i would expect you would have to get it with a bar bell

i was wanting to get my belly button done but i have been looking at pictures on the Internet and some get it cut with a knife does everyone have to get that ?
i am getting a little scared now after looking at they pictures
