I have a J2 visa that expires on Jun 2005 and a working permit that expires on Jun 2004.  The question is: What do I have to do to renew the working permit for the rest of the duration of my visa?

You have to proceed as if you were applying for Employment Authorization for the first time. Best is to start the process over again three months before your current employment authorization card expires.
