No. There's been no real consistency other that people using the headers
as documented. Initially it was the case that all hyphenated auxiliary
headers had the second word lowercased. I think I was the guilty party
that introduced the first auxiliary header (Patch-To: in comp.sources.misc)
that did not follow this.

Nope. People simply try to use the documented format of the header. Yes,
auxiliary headers can be in any 'case' but for the most part it is better
to try and follow the documented format. Right or wrong, there is a lot
of software out there that parses news article headers, and not all of it
recognizes case correctly.
