Pseudo-index to faq-maintainers archives (through November 1994)

Denis McKeon <galway@chtm.eece.unm.edu> took some time to pseudo-index
the faq-maintainer mail archives through November 1994. Messages are
grouped by their Subject line, and then by general topic. This should
be helpful to those who wish to research a particular topic -- rather
than having to wade through the raw archives, they can search for the
subject lines of the topic(s) which they are interested in.
