In some aspects, a user indicates by way of a user interface a type of patient complaint and selects a type of medical record form that is to be used in conjunction with examining the patient. The system guides the user and accepts the user's inputs and provides a suitable medical record form from a plurality of form types. The selected form is printed, and used by the medical staff in the examination of the patient.
