The body includes a shell having a cavity to receive the instrument so that the tip extends past the shell, a bookmark extending away from the shell asymmetrically, the bookmark having a generally flat surface for removably connecting one or more tags, and a latch. The cap includes a bore for receiving the instrument, and a catch received by the latch so that the cap is selectably positionable in two positions.
