I use a perl script originally announced by someone from Germany on this
list (and whose name I have regretably lost), much modified since by Doug
Brink and Phillip Walker, to accomplish this task for the
soc.org.nonprofit FAQ. It puts the table of contents into both the .txt
and the .html files.

It requires that the source file start with a line of 30 =s and that each
section be marked by a line of five -s. It isn't perfect, but it's been
running well for me through several iterations. I'll be glad to forward
it to anyone who's interested. (I'm diffident, though, because I didn't
write it and don't understand it.... just use it as a black box.)
