Lucio Costa planned Brazil's new capital, Brasilia, and the main buildings were designed by Oscar Niemeyer. The city, which opened in 1960, is in the shape of a bird in flight with housing on the wings. Its population, including satellite towns, is 2 million, but they are hidden from view. There is a lot of space around the buidings; individual types of buildings are kept separate. Architectural monuments such as the cathedral, parade ground and Foreign Ministry are impressive but unattractive.

The Accident and Emergency Primary Care Service at King's College Hospital, London, England includes GPs in the department as specialists. Junior hospital doctors are not trained in primary medicine. A specially-trained nurse decides patients' needs. Some should be seen by a hospital doctor and some by a family doctor. One third of patients were found to need primary care. The department has seven GPs working between 1pm and 9pm every day. They train hospital doctors in patient-centred skills.

The case of a 10-year-old girl who needs leukaemia treatment costing 75,000 pounds sterling has highlighted the very difficult decisions which must now be made about the allocation of limited healthcare resources. The general public is now becoming aware that the National Health Service cannot afford to pay for some types of medical treatment. There are now heated debates about who bears the ultimate responsibility for deciding whether a patient should receive treatment or not.
