Lack of supervision costs Balfour Beatty Rail Maintenance Ltd 500 000 pounds sterling fine over Rivenhall derailment
Article Abstract:
The UK Health and Safety Executive launched an investigation after the last eight vehicles of a 21 vehicle Freightliner train were derailed near Fox Lane Bridge, Rivenhall, England, on Sep 23, 1997. This incident led to a court case, and Balfour Beatty Rail Maintenance Ltd has now been fined 500,000 pounds sterling after pleading guilty to breaching Section 3(1) of the Health and Safety at Work Act. This is the highest ever fine resulting from a railway accident or incident.

Shattered rail caused Hatfield train detrailment
Article Abstract:
Investigation has shown the train deraliment on Oct 17, 2000, in Hatfield, Hertfordshire, was caused by fractured rails that shattered under stress. Thirty-five meters of rail in one location and 54 meters in another fractured into more than 300 pieces, killing 4 passengers and injuring 70.
