Unconditional discharge

Article Abstract:

A pilot study of 20 psychiatric patients has revealed inadequate communication of information in the discharge process. Thirteen patients had not been told how to obtain future help. Less than half those requiring follow-up appointments had been informed about arrangements. Only one patient appreciated possible side effects of medication or knew about support organisations. Only seven understood the procedure for acquiring medical certificates. The survey has prompted the introduction of information leaflets and production of a discharge checklist.

Discharge to the community of older patients from hospital

Article Abstract:

Several studies have emphasised the problems faced by older people when they leave hospital, although care in the community is seen as a positive alternative to hospital care. Some studies claim that older people are often discharged when they still require extensive rehabilitation, but others welcome early discharge provided detailed discharge planning is undertaken.

Article Abstract:

Issues are presented concerning the management of a recuperation and rehabilitation scheme which has been established by the social and healthcare services in North Yorkshire. The benefits to patients are discussed.
