There are now around 300 federal Indian reservations in 33 US states, but the land is often poor and facilities inadequate. On some of these reservations, infant mortality rates are higher than average, and sudden infant death syndrome (SIDS) can be four to five times more common than in the US as a whole. Health promotion material must use images and words which will be understood by the native American people, and health care services must be sensitive to the patients' cultural background.

The Mental Health (Patients in the Community Bill) has recently had its first reading in the House of Lords. The Bill, which is an amendment to the Mental Health Act, will allow doctors, social workers and community psychiatric nurses (CPNs) to take patients to a hospital or training session for treatment if they breach a contract set out before they leave hospital. The contracts may include details of an occupation or training course, treatment appointments, or simply a particular address.

Sexual harassment by male patients or colleagues has been suffered by up to 79% of nurses and nursing auxiliaries, with varying emotional reactions from the victims. These incidents often take place when no witnesses are present, and many are not reported, as the victim does not want to harm the family of the perpetrator. Harassment is often the result of male patients and colleagues misinterpreting the nurse's friendly approach.
