Severe ovarian hyperstimulation in a spontaneous singleton pregnancy

Article Abstract:

Ovarian hyperstimulation syndrome (OHS), an abnormal increase in size of the ovaries, is an often serious complication of ovulation. When medical problems such as trophoblastic disease are not present, OHS is usually mild. This study examined a severe case of OHS that occurred in a 16-year-old pregnant female who had no other medical problems. She initially complained of abdominal fullness and shortness of breath and reported having gained 27 pounds in the prior two weeks. Physical examination revealed that her abdomen was severely distended. Ultrasound imaging examination revealed ascites, or accumulation of fluid in the peritoneum, and enlarged ovaries. She was thought to still have a viable pregnancy. Further testing showed no cancer or other abnormalities, yet she continued to suffer from abdominal pain. Examination following a therapeutic abortion showed normal fetal tissue and normal placental tissue. When symptoms continued following the abortion, the diagnosis was changed to a severe case of hyperstimulation. After 10 days of treatment, the ascites resolved and she lost 30 pounds. Three weeks later her ovaries returned to normal size. This was the first reported case of severe OHS occurring during a spontaneous pregnancy where no other medical problems existed. Spontaneous closure of the hymen during pregnancy

Article Abstract:

Secondary closure of the hymen is reported in 2 cases, both of which occurred subsequent to surgical procedures that involved the hymen. An interesting case of spontaneous formation of an imperforate hymen during pregnancy in the absence of previous surgical procedures is presented.
