Article Abstract:
A nursing award winner describes her rheumatology unit. Her health trust decided to launch a nurse-led dedicated rheumatology unit in 1993. The decision was a response to factors including reduction of junior doctor hours and problems related to the distribution of rheumatology patients throughout the hospital. The unit has 10 beds and the lead nurse must have a master's degree in clinical nursing. Nurses carry out all duties previously performed by junior doctors. They follow a care pathway governing interventions from preadmission to discharge, and continuously monitor effectiveness.

The requirement for expert clinical decision-making in the role of nurse consultants is reviewed. An attempt is made to explore the experiences and views of consultant practitioners in order to gain a better understanding of the role.
