Devising a nurse-led home blood transfusion service

Article Abstract:

It is possible for a rapid response team to routinely administer blood transfusions in patients' homes on the basis of a protocol that allows the consultant to retain medical responsibility and makes patient safety the top priority. Patients who receive blood transfusions in their own home do not need to make inconvenient hospital visits, and the home-based procedure is also significantly less expensive than in hospital. Patients remain in control of the process, and are able to choose a convenient time.

Significant improvements have been made by the national blood services in the UK to ensure the safety of red blood cells and blood components. There are a number of ways in which nurses can prevent blood transfusion errors. These include ensuring that patients having transfusions have a name band bearing all their details.

It is vital to be aware that there are many different forms of reaction to a blood transfusion. Patients must be regularly monitored during the transfusion and their responses compared with pre-transfusion vital signs. The reactions can include protein reactions, infective shock and haemolytic reaction.
