Ten years after the failure of president Clinton's Health Security Act (HSA), the Unites States continues to face multiple stresses in health care. Neither incremental legislation nor private sector changes in health care organization, and financing during the past decade are ameliorating the problems addressed by the HSA, thus making ways for new troubles to emerge.

He states that safety can be achieved only in an environment where errors could be openly identified and studied.
