Article Abstract:
The role of physicians in managing health care facilities is discussed. No systematic strategies for involving physicians in managerial positions have been detected. However, the 'clinical directorate' model developed at Johns Hopkins University gives doctors a chance for playing the role of administrators of health facilities. The clinical directorate delegates physicians as heads of unit with its own budget allocation. They are accountable for financial as well as clinical outcomes.

Article Abstract:
Medicine as a profession is experiencing a significant decrease in its traditional autonomy. The decrease in the professional autonomy of physicians creates a commensurate increase in the degree of control exerted over them by the health care organizations in which they work. An analysis of the factors eroding the autonomy of the medical profession and organizational categories for alternative models of health care delivery are presented.

Medicine as a profession is undergoing vast changes. A fictitious character based on interviews with physicians is used to describe the various career possibilities being considered and explored by physicians. Being a medical director, buying a business and establishing a joint venture are ruled out. Managed care seems to be a viable alternative.
